There are two pressings of this LP.
This is the first pressing with the "monster" cover. The second pressing has the "burned man" cover. There also exist a small number of copies in hand made sleeves.

Included is a one sided card stock insert with images and discographies for all artists, plus a sheet with notes on AMM by Eddie Prévost.
The portrait used for Nurse With Wound on the insert is that of John Cage.
