This version of the release has the barcode as an actual sticker on the disc, with an address for Marina Del Rey, CA on it, which varies from the address located on the back of the insert which lists Los Angeles, CA.

The release has no back cover. The disc is a picture disc that shows the track listing, as well as having the barcode on the disc. On new copies the disc is flipped top-side towards the back of the case, similar to European singles. So in essence, the disc label is the back cover.
